How to fill DS-157 Application Form

Question pattern is same as how they are appearing on DS-157 application form. You can easily refer the same number with the form.
( ) - Indicates that possible answers.
[ ] - Indicates that explanations by me.

1. Last Name [As in Passport].
2. First Name [As in Passport].
3. Full Name in native alphabet [Optional].
4. Clan or Tribe Name [Not required].
5. Spouse’s Name [As in Passport].
6. Father’s Full Name [As in Passport].
7. Mother’s Full Name [As in Passport].
8. Full name and address of Contact Person or organization in the United states [You can specify the M.D name of the consultancy & U.S Office address].
9. List all the countries you have entered in the last ten years [If U.S is the first country you are going then mention NA otherwise give the YEAR and COUNTRY name].
10. List all the countries that have ever issued you a passport. [If only one then specify INDIA otherwise list the country names].
11. Have you ever lost a passport or had one stolen. (YES/NO).
12. Not Including current Employer, List your last two employers. [Mention 2 employer’s details and don’t include the present employer].
13. List all professional, social and charitable organizations to which you belong (belonged) or contribute (contributed) or with which you work (have worked). [Just mention the present employer name].
14. Do you have any specialized skills or training, including firearms, Explosives, Nuclear, Biological or chemical experience? (NO).
15. Have you ever performed military service? (YES/NO).
16. Have you ever been in an armed conflict, either participant or victim: (YES/NO)
17. List all educational institutes you attend or have attended. Include vocational institutions but not elementary schools: [Mention from 10^th class onwards till your higher degree – you can mention ‘Dates of attendance’ approximately as in your certificates].
18. Have you made specific travel arrangements? (YES/NO) [Of course it would be NO]
